Congregate Meals: Serving meals at four locations in the county. Meals are for senior citizens age 60+
and their spouse. Meals are nutritionally balanced and healthful meals in a congregate setting with plenty of
fellowship. Diabetic meal available. Age 60+ a suggested donations of $2 per meal
Home Delivered Meals: Delivered to homebound clients. Special diabetic menu. Seven days per
week availability with some being frozen meals. Age 60+ a suggested donation of $2 per meal.
Homemaker: Two hours per week service provided including, laundry, dusting, vacuuming, dishes,
shopping, etc. A suggested donation for service is based on income.
Personal Care: One hour twice a week. Bathing, grooming, hair, assisting moving and making the bed.
A suggested donation for service is based on income.
Respite: Four hours per week to allow the caregiver time to run errands or to have some time to relax and
enjoy themselves. Includes assistance of daily living for the client. There is also funding available for
Alzheimer’s Respite. A suggested donation for service is based on income.
Care Management: A program for persons needing additional service. A nurse does an assessment and
determines the needs of the client. A nurse evaluates every three months.
Telecare: A service from Tawas St. Joseph Hospital. Client applies and someone from the volunteer staff
calls every day to make sure the client is okay. This is particularly helpful for people not having someone
checking on them on a regular basis.
Advanced Directive: Forms are available to state your wishes about life support, etc. in the event you
are unable to do so or your Power of Attorney is unavailable.
CSFP Commodities: Based on income, these are available the first Monday of every month for age
60+, pregnant women and children under 6 years of age (not on WIC program). Distributed at Lincoln
Senior Center.
TEFAP: Every three months based on income. Nov., Feb., May and Aug. Food items vary each
distribution. Distribution at Lincoln Center.
Volunteers: Transporters and Chore Service - includes taking clients to appointments and minor home
repairs and yard work.
MMAP: Medicare Medicaid Assistance Program - a free health-benefits counselling service for
Medicare and Medicaid beneficiaries and their families or caregivers.
